Responsible for:

The provision of high quality, flexible and safe person centred healthcare and personal care to customers. This can be in their homes and in the community. This may include complex healthcare, practical, emotional and personal care with an emphasis on helping customers achieve a fulfilling life and maintain their independence wherever possible.

Working to protect our clients and families who we come into contact with, safe from harm and abuse.
Working with customers and other professionals to support the person to achieve the outcomes in their personal care plans as appropriate to each person.

Adhere to Care Visions At Home policies and procedures, Care Quality Commission standards, legislative requirements and any other relevant regulations.

Undertaking training and development required, look for ways to improve the way you do our work, willingness to learn from others and share knowledge and best practice to enhance Care Visions At Home Services. Achieve and maintain core care skills and standards.

Key Tasks :

  • To work as part of a team of health care assistants to promote and be supportive to the personal care and independence of Care Visions At Home customers in line with Person’s personal care plan. This can be complex health care interventions as well as standard personal care. Be positive and helpful to those we care for respecting individuality.
  • To support and update the customer’s personal care plans to meet the Person’s needs and goals.
  • To attend customers reviews and provide feedback where appropriate.
  • Communicate in a straight forward way with our clients and colleagues showing respect, listening to what they have to say and managing the feedback with other relevant care professionals to maintain high care standards.
  • Encourage customers to influence the development of the service to ensure that it reflects their views and ideas.
  • To develop effective working relationships with the other employees within the service working to an open, positive and inclusive working culture.

  • Participate in evaluation of Care Visions At Home against agreed organisational goals, business, and quality objectives.
  • Participate in the updating of Care Visions At Home management information systems.
  • To work alone and with others in customer’s homes and in the community, ensuring your own personal safety by working to the organisations policies and procedures. Respecting the position of trust you have with the people we support.
  • To attend staff team meetings and staff training days as required.
  • To cooperate and participate in supervision with line management.

  • Systematically solve day to day problematical issues which arise.
  • To comply with the no smoking policy.
  • To ensure that Health and Safety is observed, engaging in safe working practices, and comply with all Policies and Procedures and relevant care standards, minimising risk to colleague’s clients.
  • Implement action to meet and maintain care standards.
  • Encourage innovative methods for the delivery of care, participate in coaching and mentoring opportunities, identifying opportunities for training and development.
  • To undertake any other duties identified by the line manager.
  • Act in a flexible and proactive way responding calmly to the day’s activities, which will mean flexible hours including evenings, nights and weekends.

General:

  • Must have minimum QCF Level 2 (previously NVQ Level 2) in Health & Social Care or willing to work towards the qualification.
  • The successful applicant must be committed to upholding and Safeguarding adults, protecting their rights, and working co-operatively to create a very high standard of care for customers receiving services from Care Visions At Home.
